The Florida School of Addictions
Studies proudly accepts Visa, Mastercard, and Discover Cards
as methods of payment. At this time, we do not accept American Express.

If you are registering online, you will be asked for your
Credit Card number before your transaction is complete. If you attempt to pay with an
American Express Card, you will receive an error message.
If you are mailing your registration paperwork, please remember to provide
your card type, number, Expiration date, CVV Number, and Card holder name in the
indicated fields on Page Two of the Registration Form.
All checks must be made payable for the exact amount
to the Florida School of Addictions Studies, be completed and signed in ink.
No Second Party or counter checks will be accepted. Checks returned for non-sufficient
funds will be subject to a $30 returned check fee.
The Florida School of Addictions Studies will send you a receipt for your
payment once the check has been received. If you do not receive a confirmation letter
confirming the receipt of your payment, please contact
the Operations Manager with the date of the check, check number, and reason for payment
(preferably an Invoice or Sales Order Number).
If you plan to pre-register and pay with a check, please print and complete the
Registration Form for the school of your choice and mail it to us with your Check.
Purchase Orders can only be used by companies, not by
individuals doing business with the FSAS.
If you are registering online on a Purchase Order, please indicate the Purchase
Order Number in the designated field on the payment screens or provide it in the

your company's name.

If you were awarded a scholarship, you should have
received a Coupon Code in your Award Letter that was mailed/e-mailed
to you. When registering online, you will be asked to enter your coupon code on
the payment screen. Please enter your Coupon Code into the designated box to
use your scholarship.
If you are mailing your registration form, please
remember to write your Coupon Code in the Coupon Code field on
Page Two of the Registration Form.
Cash is only accepted as a method of payment for on-site registration
and on-site purchases (auctions, books, FSAS merchandise, Dinner Dance and Auction
Cash is not accepted for mail-in registrations or payments. If you
do not have a Credit Card or Checking Account, we also accept Money Orders
as long as they are made payable for the exact amount to the Florida School
of Addictions Studies. No Second Party or Counter checks are accepted.
If you or your organization received a Coupon Code, you can enter it
into the Coupon Code field on the Payment Screen while using the Online Registration
to apply your discount.
If you have a question about a Coupon Code you received that requires activation,
please contact the Operations Manager.
If you are mailing your registration forms and have a Coupon Code, simply indicate
the Coupon Code in the Coupon Code Field on Page Two of the Registration Form.
Refund requests made 7 or more days before
the paid for training event will be subject to a $25 Administrative Fee. Requests
for refunds received within six days of the paid training event will require written
FSAS Board approval and will be subject to the $25 Administrative Fee.
