2008 FSAS SCHOLARSHIP INFORMATION

DATES: Sunday, July 20 through Thursday, July 24, 2008

LOCATION: University of North Florida, Jacksonville, Florida

The Florida School of Addictions Studies is pleased to announce that a limited number of scholarships are again available for students who demonstrate financial need and meet the criteria specified by the various agencies who donate scholarship funds. Scholarships include full tuition, breakfast & lunch Monday through Wednesday, and breakfast Thursday, as well as Tuesday evening’s dinner and special activities. Scholarships DO NOT INCLUDE the cost of dorm or hotel rooms for the four days.

Although some changes may occur, the planned course offerings include:

About the 
PROFESSIONAL OF THE YEAR AWARD

27th Annual Florida School of Addictions Studies

  July 20 through July 24, 2008, University of North Florida
Jacksonville, FL

 

Since 1983, the Florida School of Addictions Studies, Inc., has annually recognized an outstanding professional in the field of substance abuse treatment and/or prevention.  This effort is to promulgate a feeling that the student body is very important to the leaders, supporters, and sponsors of the School.  Nominations for this prestigious award are encouraged from any and all interested parties, including the FSAS Board or Directors and the FSAS Membership Committee; however, current/former members of the FSAS Board of Directors are not eligible for this award.


The purpose of this award is to honor a person who has worked in the addictions field for a minimum of five years.  It is designed to be awarded to the professionals who provide direct services to consumers and who have consistently contributed to the field of addictions, distinguished themselves beyond the requirements of their jobs, and demonstrated excellence in the field.  This effort is to promulgate a feeling that the student body is very important to the leaders, supporters, and sponsors of the School and that the students will take pride in knowing that one of their peers has been chosen for their contributions and professionalism.

 

 

Nominations for this honor are encouraged from any and all interested parties, including the FSAS Board and the FSAS Membership Committee. Completed nomination forms should be sent to the Chair of the Scholarship and Awards Committee. Current/former members of the FSAS Board of Directors are not eligible for this award.

 

The deadline for nominations for this award shall be published in the FSAS newsletter and/or by mailings, including electronic, in accordance with the Department of Children & Families (DCF) contract requirements. Nominations will not be accepted after the deadline unless the FSAS Board votes to extend the deadline. The FSAS Board members shall make every effort to solicit qualified nominations for this award.

 

The Scholarship and Awards Committee shall review nominations no later than one week prior to the December Board meeting. The committee shall recommend a winner, a first runner-up, and a second runner-up. All qualified nominations shall be presented to the FSAS Board for review. The final decision for the winner and runner-ups shall be by the Board by a majority vote of those present at the meeting. The by-laws of the FSAS do not provide for a proxy vote.

 

Recipients of the award receive a plaque, a full tuition scholarship for the regular school, and a $500.00 cash award. This will be presented at a plenary at the school and the recipient must attend the full school to be eligible for this award. If the winner is not able to attend, the pre-selected runner-up will be declared the winner. In case of a valid, last minute emergency, the board may make an exception for the non-attendance. Exceptions shall only be allowed for personal or family illness, death of a close family member, or written request from an employer as to a job related situation that prevents the winner from attending. In such circumstances, the Board will decide, by majority vote, what part of the compliment will be presented.

The award winner must furnish, on a timely basis, a 5-inch by 7-inch or larger photograph suitable for reproduction. The recipient shall bear the expense of transportation, lodging, and any meals not furnished at the school.

ELIGIBILITY CRITERIA:

·         A person who has worked in the addictions field for a minimum of five (5) years

·        For those professionals who provides direct services to consumers and have consistently contributed to the field of substance abuse, having distinguished his/her self beyond the requirements of their position and demonstrated excellence in the field. 

·         Available to attend the 2008 School July 20 through July 24, 2008

·         Current/former members of the FSAS Board of Directors are not eligible for this award

 

DOCUMENTATION REQUIRED:

·         Complete Nomination form and nominator’s letter setting forth justification for the nominee’s selection as Professional of the Year

·         Current Resume of the person being nominated for Professional of the Year

·         Two (2) letters of recommendation from nominee’s colleagues, peers, supervisors, clients, or informed others who can attest to nominee’s achievements.
